The Office of Student Conduct completes institutional behavioral background checks for students applying to other institutions or for employment. Please allow a minimum of 24 hours for all forms to be completed. All forms must be signed in the presence of a staff member in the office of student conduct.
All background forms must be accompanied by a release from the student allowing the Office of Student Conduct to share the information requested with the other institution or potential employer. If this is not provided by the company, you can use the one below.
